Documents requirement for D4 Test:

  • D4 Medical Form (available on gov.uk)
  • Photo ID, passport, driving license.
  • Your driving glasses/contact lenses & prescription.
  • Medical records relevant to medical history & prescription.

What Tests Are Expected On Appointment Day

A cuff will be applied to your arm to take recordings.

VISION CHECKUP

You will have to read a lettered chart (snellen’s chart) from 6m/20feet.

COMPLETE HISTORY

A clinician will record your history on proforma provided by DVLA.

*All HGV drivers have to bring full medical history from GP surgery to obtain the medicals. (NHS App Summary is not applicable)

At D4Medics Drivers Solution, we offer clear and transparent policies regarding refunds and cancellations.

CANCELLATION POLICY

We have a 48-hour cancellation policy. If you miss your appointment, or cancel with less than 48 hours’ notice, you will be charged the full price of your appointment.

If any scheduled appointment is cancelled within 48 hours or more before the appointment, 50% of the fee will be refunded.

REFUND POLICY

Any refund which is due will be refunded into the client’s account within 5 to 10 working days of notification.
