A crane operator medical is a medical examination that is mandatory to any person working on cranes or a heavy machinery operator, even a forklift operator. - Existing operators need forklift medical tests and regular examinations. Most operators below the age of 65 should have it at least once every 1 to 3 years as per employer policies and the HSE instructions.
- Annual assessments are typically required from age 65 onwards to ensure continued fitness for safety-critical roles.
Compliance: All operators must comply with employer health requirements, HSE safety standards, and DVLA-aligned medical fitness standards for safety-critical machinery operation.
Forklift and Crane Operator Medical Requirements
To have a smooth and comprehensive assessment, the following documents should be brought to their medical appointment:
- Crane operator medical form or forklift medical form, filled in as employer or HSE requirements.
- Valid photo ID.
- Driving glasses or contact lenses with prescription.
- All the information regarding any medical conditions that you have and the medications you are taking and the hospital consultants with whom you regularly interact with.
- Height, weight, smoking and alcohol history (it will be helpful to bring a recent note with your measurements to the doctor).
Note: Operators who have heart conditions or other appropriate health conditions may need further examination, exercise test or an echocardiogram, prior to their crane or forklift medical evaluation.

What Tests Are Expected On Appointment Day
A clinical blood pressure measurement using a calibrated cuff is performed to satisfy DVLA standards.
Vision is assessed utilizing a Snellen chart at the specified distance of 6 metres (20 feet).
A clinician records your medical history on the official DVLA D4 form as part of the medical assessment.

CANCELLATION POLICY
Appointments must be cancelled a minimum of 48 hours in advance. Missed appointments or cancellations within 48 hours will be charged the full fee. Cancellations made 48 hours or more in advance will be refunded 50%.

REFUND POLICY
Any refund due for your D4 medical appointment will be processed to your account within 5–10 working days after notification.
