- Valid photo ID
- Driving glasses or contact lenses with prescription.
- All the information regarding any medical conditions that you have and the medications you are taking and the hospital consultants with whom you regularly interact with.
- Height, weight, smoking and alcohol history (it will be helpful to bring a recent note with your measurements to the doctor).
